Rappahannock Health Corporation is the charitable organization behind US-LTN — the United States Local Triage Network. Through our programs and the network's physician-led local clinics, we bring daily health monitoring, lifestyle medicine, and community-based clinical care to underserved and rural communities.

The mission of Rappahannock Health Corporation is to empower underserved communities through comprehensive disease education, the facilitation of local wellness programs, and the promotion of public health awareness to help individuals build sustainable, healthier lives.
Our clinical services — home health monitoring, cellular telemetry, and lifestyle-medicine programs — are delivered in furtherance of this charitable mission, not for private profit. Program revenue, including Medicare reimbursements, is reinvested in patient monitoring equipment, local clinical staffing, and community wellness and education programs.

We carry out our mission through US-LTN, our flagship program, and its physician-led clinics: Litchfield Health in Connecticut, Rappahannock Health in Virginia & North Carolina, and Atlantic Telehealth in New Jersey & Massachusetts.
Our flagship program: daily monitoring, lifestyle medicine, and community support for patients managing chronic disease in rural America. US-LTN operates under the Corporation's federal tax exemption.
